Our Shared Living Services focus on fostering long-term, meaningful relationships through the shared experience of daily life. Our model creates a supportive and caring household environment, where participants live with individuals or families from the community — referred to as Shared Living Hosts — who provide both companionship and direct care support.
Our program overview
Inclusive Living Environment
Participants become integral members of their Shared Living Host’s household while receiving support to develop, maintain, or enhance the skills necessary for independent living and active community participation, including employment if desired.
Home-Based Setting
Services are delivered within the home of the individual or host family, promoting stability, comfort, and a sense of belonging.
Comprehensive Support
Shared Living Services include nursing case management and delegation services. Additional service hours may be authorized based on the participant’s individualized needs, as outlined in their Person-Centered Plan and Health Risk Screening Tool (HRST).
A completely customized assistance plan
The nature and extent of assistance, support, and guidance provided through Shared Living are determined by the participant’s assessed needs — encompassing physical, psychological, and emotional support — and are outlined in the participant’s Person-Centered Plan. All support is delivered to promote autonomy, honor personal preferences, and help the participant achieve their individually defined outcomes.
